Welcome to Bhalla Endocrinology

We’re honored you’ve chosen us for your endocrine care. To ensure a smooth and efficient experience at your upcoming new patient appointment, please review the instructions below:

1. Complete Your New Patient Forms in Advance

Your new patient forms will be emailed to you after your appointment is scheduled. These can be conveniently completed and signed on your computer or smartphone.

To save time on the day of your visit and help us prepare for your care, please complete all forms in full prior to your appointment.

If you are unable to complete the forms ahead of time, we kindly ask that you arrive at least 20 minutes early to complete them in the office. This helps prevent delays in your scheduled appointment.

Please note: If you arrive after your scheduled appointment time and your forms have not been completed in advance, your appointment will need to be rescheduled.

2. If You Were Referred by a Physician

Please ask your referring physician’s office to fax your medical records at least one week before your visit to:

3. Confirm Your Appointment Online

You will receive appointment reminders starting one week prior to your visit. Please confirm your appointment using the link in the reminder to ensure your time is reserved.

4. New Patient Appointment Confirmation Policy

To ensure timely access to care, we require all new patient appointments to be confirmed at least 48 hours in advance.

If we do not receive your confirmation, the appointment will be cancelled and offered to a patient on our waiting list.

This policy helps reduce no-shows and allows us to better serve patients eager to be seen. Thank you for your understanding and cooperation.

5. What to Bring With You

Please bring the following items to your appointment:

✅ Driver’s License or Photo ID

✅ Insurance card(s)

✅ Referral from your Primary Care Physician (if required by your insurance)

Note: Failure to obtain a required referral will result in denial of insurance coverage and full patient responsibility.

✅ Relevant lab and diagnostic reports

If your prior records are available via an online patient portal, please bring your login username and password so you can access them during the visit.

✅ An up-to-date list of all medications, including doses and frequency

If you are unsure of your medications or doses, please ask your primary care office to fax us your medication list at least one week prior to your visit.

✅ If you have diabetes: bring your blood sugar log and glucometer

✅ A form of payment for any financial responsibility due at the time of visit (credit card, cash, or check)
